Log into our secure portal to fill out required patient information prior to your consultation at our office. Please call our office if you do not have a username and password.

Office Policies

Consultations are made by appointment only. Our office staff may be reached Monday-Friday 800 AM to 500 PM @ 303-872-8250.Office hours are M-F 8-5, with lunch from 12-1.

If you need to cancel your appointment, please provide 24 hours notice.

We currently accept cash, personal check, Visa, Mastercard, Discover or American Express as forms of payments. We are pleased to offer information on financing options through M-lend. Please contact our office for an application.
